Job summary

Suburban Health at Home is seeking a Registered Nurse in home health to manage patient care plans and provide one-on-one care to adult/geriatric patients in the home setting. You will perform wound care, infusion therapy, medication administration, and educate patients and families, typically completing 2 admission visits or 6 routine visits per day.

Qualifications

• Associate’s degree in nursing from an accredited college/university.
• Current license from the Indiana State Board of Nursing.
• At least one year’s experience in nursing.
• Must possess valid Driver’s License and automobile insurance.

• Manage and evaluate patient's care plans, providing wound care, infusion therapy, and education to patients and families.
• Perform medication administration and education to patients and families.
