Job summary

Suburban Health at Home in Greenfield, IN is seeking a Weekend Admissions Nurse to join our home-based care team. You will manage your own schedule and perform Start of Care assessments for adult and geriatric patients in the home setting.

Qualifications

• Associate’s degree in nursing from an accredited college/university.
• Must hold Indiana State Board of Nursing license.
• At least one year of home health nursing experience.
• OASIS experience required.
• Must possess valid Driver’s License and automobile insurance.

Responsibilities

• Perform initial Start of Care assessment for adult/geriatric patients.
• Conduct ongoing assessments and routine wound care visits.
• Travel to patient homes and coordinate care with families and healthcare partners.
