Job summary

WesleyLife is seeking an RN with home health experience to join our team and support clients in their own homes across central Iowa.

As a Visit Nurse, you will perform in-home nursing assessments, counsel families, and collaborate with the interdisciplinary team to ensure high-quality, person-centered care. A valid RN license is required and pre-employment health screening is part of our process.

Qualifications

• Active RN license required.
• Experience in home health, wound care, infusion therapy, or catheter care preferred.
• Ability to build meaningful connections and provide person-centered care.

Responsibilities

• Perform comprehensive in-home nursing assessments including recertifications and routine visits.
• Evaluate client progress and adjust care plans as needed.
• Counsel clients and families on health conditions, medications, and care plans.
• Communicate with the Nurse Case Manager about orders and changes in condition.
• Collaborate with the interdisciplinary team to ensure coordinated care.
