Job summary

Hueman is actively hiring Nurse Practitioners to perform in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around West Milford, NJ. This role offers flexible scheduling and travel-friendly opportunities, letting you contribute to community health from patients' homes.

NPs will not treat or prescribe in-home during HRAs but will gather data and educate on wellness strategies. The position supports comprehensive home assessments with training, scheduling assistance,

Qualifications

• Master’s degree or commensurate NP experience.
• Active NP license with state authorization.
• National board certification for NP practice.
• BLS certification is required.
• 12 months of paid advanced clinical provider experience preferred.

Responsibilities

• Travel to and from patients’ homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventive healthcare measures.
