Hueman is hiring Nurse Practitioners to conduct in-home Health Risk Assessments for Medicare/Medicaid members in and around Passaic, NJ. NPs will travel to patients’ homes and perform comprehensive assessments, without prescribing in the home.

The role offers flexible scheduling, travel reimbursement, and comprehensive training to support safe, high-quality patient interactions in a home setting.

Qualifications

• Master’s Degree in Nursing (NP) or commensurate experience.
• Active NP license in the state.
• National board certification as NP.
• Current BLS certification required.

Responsibilities

• Travel to patients’ homes for HRAs.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and document findings accurately.
• Provide basic patient education on wellness and preventive care.
