Job summary

Hueman is actively hiring Nurse Practitioners to complete in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around South Hadley, MA. This opportunity adapts to your lifestyle, offering a flexible schedule and the chance to earn supplemental income or deepen community ties.

As an HRA NP, you will travel to patients' homes, perform non-invasive assessments, review medical history, document findings, and provide basic wellness education.

Qualifications

• : Master’s Degree in Nursing or commensurate experience with NP license
• : Active State NP license
• : Active National Board Certification
• : Current BLS certification
• : 12 months of paid advanced clinical provider experience preferred

Responsibilities

• Travel to and from patients' homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and document findings.
• Conduct medical reconciliation to ensure patient safety.
• Provide basic patient education on wellness and preventive healthcare measures.
