Hueman is seeking Nurse Practitioners to perform in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Washington, PA. This role offers travel to patients’ homes, a flexible schedule, and comprehensive training to support effective, person-centered care outside traditional clinics.

Qualified NPs with an active license will interact with members in a home setting, gather medical histories, and provide education on wellness and preventive healthcare while ensuring

Qualifications

• Master’s degree in nursing required and active NP license.
• Active State NP license required.
• Active National board certification.
• Current BLS certification.

Responsabilités

• Travel to and from patients’ homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate and timely documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventive healthcare measures.
