Qualifications

• Current license to practice physical therapy in the assigned state.
• Graduate from a CAPTE-accredited physical therapy program.
• 1+ year of physical therapy experience in home health care.
• If less than 1 year of experience, regional clinical leaders' approval is required and mentoring/residency participation.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct reexaminations (every 30 days) and discharge planning; modify as needed.
• Other duties as assigned.
