Job summary

UnitedHealth Group is hiring a Physical Therapist to provide skilled PT services in patients' homes, promoting mobility, independence and safety. You'll collaborate with patients, caregivers and an interdisciplinary team to support recovery and prevent hospitalizations.

The role requires a PT license in the assigned state, CAPTE-approved education, and CPR certification, with 1+ year home health experience preferred; mentoring option if less than a year is required.

Qualifications

• Current license to practice physical therapy in the assigned state.
• Graduate from an accredited physical therapy educational program as set by CAPTE.
• 1+ related physical therapy experience for home health care to ensure readiness for in-home services.
• If less than one year of experience, approval from regional clinical leaders and mentoring or residency participation.
• Current CPR certification.

Responsibilities

• Maintain, restore and improve movement, activity and function.
• Provide interventions, conduct reexaminations at least every 30 days, modify plans, and perform discharge planning.
• Other duties as assigned.
