Signify Health, part of CVS Health®, seeks a Per Diem Physician to provide in-home health evaluations across participating regions. The role focuses on 45-minute visits, patient education, and coordinated care planning without prescribing or ordering labs.

You will travel locally and within licensed states, conduct virtual visits as needed, and join a national network of clinicians dedicated to improving health outcomes. This part-time position offers hourly pay and a comprehensive benefits mix.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses preferred.

Responsibilities

• Bring heart into every visit within a national clinician network.
• Visit millions nationwide to connect patients with the right care.
• Work with Signify Health as a clinician colleague.
• Be flexible to travel locally and within licensed states.
• Conduct virtual visits as needed.
• Provide Diagnostic & Preventive Services as indicated.
