Signify Health, part of CVS Health, is hiring a Full-time Physician to provide in-home health evaluations, engaging with patients in their homes and bridging gaps in care. The visit lasts about 45 minutes and includes a medication and medical history review, a physical examination, and optional diagnostic tests as ordered by the health plan.

You will join a national network of clinicians, travel locally within licensed states, and may conduct virtual visits as needed.

Qualifications

• Active, unrestricted medical license in one or more coverage areas.
• Multistate licenses preferred.
• Able to cover the following primary locations: St. Lawrence County/ Plattsburgh NY.

Responsabilités

• Conduct in-home health evaluations and document findings using the clinical workflow.
• Engage with patients to discuss health goals and care plans.
• Review medications and medical history during visits and update records.
• Perform physical evaluations and coordinate any necessary diagnostics.
• Collaborate with the patient’s health plan to facilitate referrals and care.
• Travel locally within licensed states and conduct virtual visits when needed.
