Job summary

Signify Health, part of CVS Health, is seeking a Full-time Physician to perform In-Home Health Evaluations, visiting patients at home and coordinating with health plans to bridge gaps in care.

Responsibilities include 45-minute visits with medical history review, physical exams, and additional DPS testing when ordered. You will travel locally and may conduct virtual visits, using an iPad and a streamlined clinical workflow.

Qualifications

• Must hold an active, unrestricted medical license in the coverage area.
• Multi-state licenses are preferred.
• MD or DO qualification required or preferred.

Responsibilities

• Conduct in-home health evaluations lasting about 45 minutes, including medication and medical history review, physical exam, and diagnostic tests as ordered by the health plan.
• Travel locally to Hartford and surrounding areas; flexible to travel within licensed states.
• Perform virtual visits as needed.
• Use an iPad and the clinic workflow for each evaluation.
• Provide DPS (Diagnostic & Preventive Services) as indicated by the health plan.
