Job summary

Responsibilities

• Performs evaluations, determines OT diagnoses, develops plan of care and performs occupational therapy procedures to restore function and mobility.
• Completes reassessments including OASIS assessments and updates the plan of care with the physician and team.
• Provides therapy, assesses equipment needs, documents progress, and plans discharge from therapy.
• Collaborates with a multi-disciplinary team and participates in team meetings and case conferences.
• Supervises Occupational Therapy Assistants and Home Health Aides and completes supervision documentation.
