Signify Health is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations across the United States. The role involves visiting members in their homes, reviewing medical history, and conducting physical evaluations with optional diagnostic tests as determined by the health plan.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)
