Job summary

Optum HouseCalls is seeking a Nurse Practitioner or Physician Assistant to provide annual in-home health assessments for Medicare Advantage and other health plan members. This field-based role focuses on preventive care, patient education, and improving health outcomes through collaboration with primary care providers.

Compensation for this specialty ranges from $109,500 to $164,000. Total cash compensation includes base pay and bonus and is based on several factors including education, work

Qualifications

• Active certification with ANCC or AANP for NP/PA.
• Unrestricted state license to practice, or ability to obtain by start date.
• Active driver's license and reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments.
• Eligibility to practice independently where allowed by state regulations.

Responsibilities

• Conduct in-home health assessments for Medicare Advantage and other plans.
• Educate patients on disease management, medications, and preventive care.
• Identify and close care gaps to improve health outcomes.
• Address social determinants of health and connect members with resources.
• Collaborate with interdisciplinary care team and primary care providers.
