Job summary

Optum HouseCalls is seeking a Nurse Practitioner or Physician Assistant to perform annual in-home health assessments for Medicare Advantage and other health plan members in Crawford & Phelps Counties, MO. This field-based role emphasizes preventive care, patient education, and collaboration with primary care providers.

Compensation includes a $10,000 sign-on bonus and $20,000 student loan repayment, with flexible scheduling and no evenings, weekends, or holidays.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Gerontic, or Adult-Gerontology Primary Care
• Active, unrestricted licensure in the practicing state (or ability to obtain by start date)
• Active and unrestricted driver's license
• Access to reliable transportation to complete home visits
• Ability to navigate varied home environments and perform assessments
• If required, obtain licensing board approval within stated timelines

Responsibilities

• Conduct comprehensive in-home health assessments
• Help identify and close care gaps
• Educate patients on disease management, medications, and preventive care
• Address social determinants of health and connect members with resources
• Collaborate with interdisciplinary care team
