Job summary

Pay is per completed assessment, with mileage reimbursement for travel and flexible, self-scheduled hours. Responsibilities include conducting thorough home visits, reviewing medical histories, educating patients, documenting findings, and coordinating with primary care teams to close care gaps.

Qualifications

• Active NP license in Kansas.
• Board certification in family, adult-gerontology, or related specialties.
• Minimum of 1 year clinical experience; geriatric experience preferred.
• Willingness to travel to patient homes.

Responsibilities

• Perform in-home health assessments across multiple locations.
• Educate patients on medications, disease prevention, and health maintenance.
• Document diagnoses and communicate findings for care management.
• Collaborate with primary care providers to address care gaps.
• Maintain a flexible schedule with no on-call, weekends, or holiday shifts.
