Job summary

Northeast Healthcare Recruitment, Inc. is partnering with Maine healthcare teams to recruit Nurse Practitioners for in-home health assessments on a per‑assessment basis.

You will perform home visits, review histories, conduct exams, and educate patients on medications, prevention, and self‑care. This per‑diem role offers flexible, self‑scheduled hours, mileage reimbursement, and pay of $80–$90+ per completed assessment.

Qualifications

• Availability to complete at least 30 in-home assessments per month.
• Active Nurse Practitioner license in Maine.
• Board Certification from AANP or ANCC in Family, Adult-Gerontology, or related specialties.
• Minimum of 1 year of clinical experience, preferably with geriatric patients.

Responsibilities

• Perform thorough health assessments in patients’ homes, including reviewing medical histories, conducting physical exams, and screening for chronic conditions.
• Educate patients on medication management, disease prevention, and overall health maintenance.
• Document relevant diagnoses and communicate findings to ensure effective care management.
• Collaborate with primary care providers to address any gaps in patient care.
• Enjoy a flexible schedule—no on-call, weekends, or holiday shifts required.
