Job summary

Northeast Healthcare Recruitment, Inc. (NEHCR) seeks a Nurse Practitioner to perform in-home health assessments across Greater Topeka, KS, on a flexible per-diem basis with travel.

You will conduct home visits, review medical histories, perform exams, educate patients on disease management, document findings, and coordinate with primary care providers. • Active NP license in Kansas.
• Board Certification in Family, Adult-Gerontology, or related specialties.

Responsibilities

• Perform in-home health assessments across locations (Topeka).
• Review medical histories, conduct physical exams, and screen for chronic conditions.
• Educate patients on medication management, disease prevention, and health maintenance.
• Document diagnoses and communicate findings for care management.
• Collaborate with primary care providers to address gaps in patient care.
