Qualifications

• Active state NP license is required.
• Board certification in FNP, AGNP, or ANP is necessary.
• Personal transportation must be reliable.
• Availability for a minimum of 60 hours per month is mandatory.
• New grads are welcome; no prior experience is required.

Responsibilities

• Complete pre-scheduled in-home health assessments for various health plans.
• Follow structured workflows with pre-populated forms.
• Ensure visits span multiple counties for higher earning potential.

Skills

Active, unrestricted state NP license

Board certification: FNP, AGNP, or ANP

Reliable personal transportation
