Qualifications

• Master’s Degree in Nursing or equivalent experience is necessary.
• Hold an active NP license valid in the relevant state.
• Board certification from a national body is required.
• Current BLS certification required.
• Preferred: 12 months experience as an advanced clinical provider.

Responsibilities

• Travel to and from patients’ homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history for documentation accuracy.
• Conduct medical reconciliation ensuring patient safety.
