Hueman in Mount Pleasant, SC is seeking Nurse Practitioners to perform in-home Health Risk Assessments for Medicare and Medicaid members. This flexible role lets you schedule around your preferences while making meaningful connections in patients' homes.

Applicants must have an NP license, board certification, BLS, and ideally 12 months of paid clinical provider experience; travel to homes is required, with documentation and patient education involved.

Qualifications

• Master’s Degree OR commensurate experience and NP license.
• Active State NP license.
• Active National board certification.
• Current BLS certification.
• 12 months of experience as a paid advanced clinical provider is preferred but not required.

Responsibilities

• Travel to patients’ homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate and timely documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventative healthcare measures.
