Job summary

Hueman is actively hiring Nurse Practitioners to complete in-home Health Risk Assessments for Medicare and Medicaid members in and around Medford, MA. This role offers flexible scheduling and the chance to engage with patients in a personal home setting.

Qualified NPs will hold an active State license, national board certification, and a current BLS. Masters in Nursing or equivalent experience is required, with travel reimbursement and liability coverage provided.

Qualifications

• Masters Degree OR commensurate experience and satisfactory NP licensure.
• Active State NP license.
• Active national certification as a Nurse Practitioner.
• Current BLS certification.
• 12 months of paid advanced clinical provider experience preferred but not required.

Responsibilities

• Travel to and from patients' homes for HRAs.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and document accurately and timely.
• Conduct medical reconciliation to ensure patient safety.
• Provide basic education on wellness and preventive care.
