Job summary

Hueman is seeking qualified Nurse Practitioners and Physician Assistants to conduct in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Franklin, ME. In this role, they will interact with patients and are not required to treat or prescribe during the home visit.

The ideal candidates should possess a Master’s Degree, an active NP/PA license, and relevant certifications, along with the ability to travel to patients’ homes. The company offers competitive compensation and a flexible schedule.

Qualifications

• Master’s Degree or commensurate experience with satisfactory completion of NP/PA license.
• Active State NP/PA license.
• Active National board certification.
• Current BLS certification.
• Preferably 12 months of experience as a paid advanced clinical provider.

Responsibilities

• Travel to and from patients’ homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate documentation.
• Conduct medical reconciliation for patient safety.
• Provide basic patient education on health measures.
