Your Health is seeking a dedicated Community Health Worker (CHW) for our Rock Hill, SC team. In this full-time role, you will serve as a key link between patients and crucial healthcare services, facilitating at-home visits, delivering medications, and ensuring access to medical supplies.

Qualifications

• Certified Medical Assistant, Paramedic, or Licensed Nurse preferred; certifications must be in good standing.
• Phlebotomy experience preferred but training will be provided.
• Previous home health or public health experience preferred.

Responsibilities

• Facilitate visits with therapists and providers in homes and facilities.
