Job summary

Anuco Home Health Care LLC in Chicago is seeking compassionate Registered Nurses to provide intermittent skilled nursing services in patients' homes. The RN will assess patient needs, develop plans of care, perform skilled procedures, and supervise clinical staff.

Candidates should have Medicare-certified home health experience, strong OASIS and care-coordination skills, and proficiency with Axxess Home Health EMR. Must maintain accurate documentation and ensure patient safety.

Qualifications

• Current Illinois RN license required.
• Medicare-certified home health experience preferred.
• OASIS experience preferred.
• Proficiency with Axxess EMR preferred.

Responsibilities

• Complete initial and ongoing nursing assessments.
• Perform Start of Care, Resumption of Care and OASIS assessments.
• Develop, implement, evaluate, and revise plans of care.
• Provide skilled nursing procedures and medication management.
• Educate patients and caregivers on medications, diagnoses, and safety.
• Supervise assigned LPNs and Home Health Aides.
