Job summary

BrightStar Care of Concord, Lexington and Woburn is seeking a Licensed Practical Nurse (LPN) to deliver compassionate home health care to clients in their homes across the region. Join a supportive team committed to quality patient care.

Responsibilities include administering medications, monitoring vitals, assisting with daily living activities, educating families, coordinating with RNs and therapists, and maintaining accurate EHR documentation.

Qualifications

• Active and valid LPN license in Massachusetts.
• Minimum 1 year of home health or related healthcare experience.
• Current BLS certification.
• Reliable transportation and willingness to travel to patient homes.
• Proficiency in electronic health records (EHR).

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor and document patient vital signs and overall health status.
• Assist with daily living activities such as bathing, grooming, and meal preparation.
• Educate patients and families on managing health conditions and care plans.
• Coordinate with RNs, therapists, physicians, and other healthcare professionals.
• Maintain accurate and timely documentation using EHR systems.
• Provide emotional support and companionship.
• Follow all safety and health regulations in home care settings.
