Qualifications

• Active and valid LPN license in Massachusetts (MA).
• Minimum 1 year of home health or related healthcare experience.
• Current BLS certification.

Responsabilités

• Administer medications and treatments as prescribed.
• Monitor and document patient vitals and status.
• Assist with activities of daily living (bathing, grooming, meals).
• Educate patients and families on care plans.
• Coordinate with RNs, therapists, physicians, and other healthcare professionals.
• Maintain accurate documentation using EHR systems.
• Provide emotional support and companionship.
• Follow safety and health regulations in home care.
