BrightStar Care of Concord, Lexington and Woburn is hiring an LPN for home health services across the region. You will deliver compassionate patient care directly in clients' homes, coordinating with RNs and therapists to implement care plans and ensure safety and comfort.

Qualifications

• Active LPN license in Massachusetts.
• Minimum 1 year of experience in home health or related care.
• Current BLS certification.
• Reliable transportation and willingness to travel to patient homes.
• Proficiency in electronic health records (EHR).
• Strong communication and interpersonal skills.
• Compassion and commitment to quality patient care.

Responsibilities

• Administer medications and treatments as prescribed by healthcare providers.
• Monitor and document patient vital signs and health status.
• Assist with daily living activities such as bathing, grooming, and meal preparation.
• Educate patients and families on managing health conditions and care plans.
• Coordinate with RNs, therapists, physicians, and other healthcare professionals.
• Maintain accurate and timely documentation using EHR systems.
• Provide emotional support and companionship.
• Follow all safety and health regulations in home care settings.
