Job summary

BrightStar Care of Concord, Lexington, and Woburn is seeking a Licensed Practical Nurse (LPN) to provide compassionate in-home health care across the region. You will work with a supportive team, delivering medications, monitoring health, and supporting daily living tasks in patients' homes.

Requirements include active MA LPN license, 1 year in home health, current BLS, and proficiency with EHR systems. A flexible schedule and career growth opportunities are offered.

Qualifications

• Active and valid LPN license in Massachusetts.
• Minimum 1 year of experience in home health or a related healthcare setting.
• Current BLS certification.
• Proficiency in electronic health records (EHR).

Responsibilities

• Administer medications and treatments as prescribed by healthcare providers.
• Monitor and document patient vital signs and overall health status.
• Assist with daily living activities such as bathing, grooming, and meal preparation.
• Educate patients and families on managing health conditions and care plans.
• Coordinate with RNs, therapists, physicians, and other healthcare professionals.
• Maintain accurate and timely documentation using EHR systems.
• Provide emotional support and companionship.
• Follow all safety and health regulations in home care settings.
