Job summary

Signify Health is seeking a Part-Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations across the United States. You will engage with members in their homes to bridge gaps in care and connect them with the right resources.

The visit lasts about 45 minutes and includes medication/history review, a physical evaluation, and optional diagnostic testing per health plan orders. Local travel and work within licensed states are required.

Qualifications

• Active, unrestricted NP/PA license in coverage area.
• Board certification required for NP or PA.
• Multi-state licenses preferred to enable travel across states.

Responsibilities

• Conduct in-home health evaluations with patients and families.
• Review medication history and perform physical evaluations.
• Coordinate with health plans to provide necessary diagnostic services.
• Travel locally and within licensed states as needed.
• Possibly conduct virtual visits when required.
