Responsibilities

• Engage with health plan members in their homes through in-home visits.
• Visit millions of people nationwide to connect them with the right care.
• Collaborate with Signify Health clinicians and travel locally and within licensed states.
• Conduct virtual visits as needed and provide services including diagnostic and preventive care.
