Job summary

Signify Health, part of CVS Health, seeks a Part Time Clinician (Nurse Practitioner or Physician Assistant) to perform In-Home Health Evaluations, connecting with members in their own homes to bridge gaps in care.

The visit lasts about 45 minutes, includes a review of medications and history, a physical evaluation, and optional tests ordered by the health plan. Travel locally within licensed states is required.

Qualifications

• Active, unrestricted licenses in the coverage area(s).
• Board certification is required.
• Multi-state licenses are preferred.

Responsibilities

• Conduct in-home health evaluations with patients in their homes.
• Review medication history and medical history during visits.
• Perform physical evaluations and document findings.
• Follow health plan orders for any additional diagnostic testing.
• Coordinate with Signify Health clinicians as needed.
• Travel locally within licensed states and conduct virtual visits as required.
