Job summary

ATC Indianapolis is seeking an In-Home Assessment Specialist (PRN) to cover Gary and surrounding areas. You will drive to client homes, conduct orientations, assess ADLs/IADLs, and train caregivers on the plan of care.

Position offers flexible PRN scheduling with no minimum hours, paid training, and weekly pay options. Mileage is reimbursed, and most visits occur during daytime hours Monday–Friday.

Qualifications

• Requires CNA/QMA/HHA/LPN/RN or solid home-care experience.
• Must have valid driver’s license, auto insurance, reliable vehicle.
• Smartphone or tablet for documentation.
• Background check required.

Responsibilities

• Drive to assigned homes in the Indianapolis metro and surrounding counties.
• Complete home orientations and required paperwork.
• Assess ADLs/IADLs, the home environment, and caregiver fit.
• Train the family or designated caregiver on the plan of care.
• Complete follow-up / supervisory visits as assigned.
• Enter notes the same day.
