Job summary

ATC Healthcare Services is seeking an In-Home Assessment Specialist (PRN) to serve Gary and surrounding areas. You will drive to assigned homes in the Indianapolis metro, complete orientations, assess ADLs/IADLs, and train caregivers on the plan of care. Most visits are daytime, Monday–Friday, with flexible PRN scheduling.

Qualifications

CNA, QMA, HHA, LPN, RN, or solid home-care/senior-care experience.
Valid driver’s license and auto insurance.
Background check required.

Responsibilities

Drive to assigned homes in the Indianapolis metro and surrounding counties.
Complete home orientations and required paperwork.
Assess ADLs/IADLs, the home environment, and caregiver fit.
Train the family or designated caregiver on the plan of care.
Complete follow-up / supervisory visits as assigned.
Enter notes the same day.
