Job summary

ATC Healthcare Services in Columbus, IN is seeking an In-Home Assessment Specialist (PRN) to visit clients in the Indianapolis metro and surrounding counties. You will perform ADL/IADL assessments, orient families to the plan of care, and document visits using a smartphone or tablet.

This role offers 100% PRN hours with flexible scheduling, daytime visits, and weekly pay. Requirements include CNA/QMA/HHA/LPN/RN or solid home-care experience and a valid driver’s license with reliable

Qualifications

• CNAs, QMAs, HHAs, LPNs or RNs with solid home-care experience
• Valid driver's license, auto insurance and reliable vehicle
• Comfortable visiting clients in home settings and completing paperwork
• Smartphone or tablet used for documentation

Responsibilities

• Drive to assigned homes in the Indianapolis metro and surrounding counties
• Complete home orientations and required paperwork
• Assess ADLs/IADLs and home environment
• Train family or designated caregiver on the plan of care
• Complete follow-up visits and enter notes the same day
