Job summary

Matrix Medical Network in the United States is hiring Nurse Practitioners to perform comprehensive in-home health assessments, each visit lasting up to one hour.

You’ll review medical history, medications, and risk factors, coordinate with PCPs and case workers, and deliver care at members' homes or SNFs. Flexible scheduling, generous monthly earnings, and a welcoming, supportive team are offered.

Qualifications

• Master’s Degree or commensurate experience with NP licensure.
• Current NP licensure in state of practice and prescriptive authority if needed.
• Board certification in appropriate NP specialty (AANP/AACN/ANCC).
• Current BLS, ACLS, or CPR certification.
• At least 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license and auto insurance (or ability to obtain).
• Strong computer skills and familiarity with EMR/health record software.
• Excellent verbal and written communication skills.

Responsibilities

• Perform adult/geriatric assessments including medical history and diagnoses.
• Coordinate with physicians, case managers, social workers and family.
• Provide health education, referrals and follow-up per Matrix guidelines.
• Order and administer point-of-care and lab-based testing as needed.
• Deliver services at the member’s residence, including homes and SNFs.
