Job summary

ELITE HOME HEALTHCARE LLC is seeking a Home Health Aide in Bloomington, IN to provide personal care to clients under the supervision of a Registered Nurse. You will assist with activities of daily living and home health tasks, following a Plan of Care and reporting changes as needed.

Qualifications include formal certification or competency evaluation, plus strong verbal and written communication skills. Must be at least 18 and able to read/write.

Qualifications

• Formal certification training or evidence of competency evaluation.
• Strong verbal, written and interpersonal skills.
• Must be at least eighteen years old and able to read/write and follow a written Plan of Care.

Responsibilities

• Provides personal care under RN supervision and assigns care to clients as directed.
• Performs activities of daily living and household tasks essential to home health.
• Assists in administering medications that are self-administered under supervision and policy.
• Reports changes in patient condition to the RN and documents care provided daily.
• Maintains infection control and safety, and communicates with the interdisciplinary team.
• Ensures confidentiality and participates in in-service programs as required.
