Job summary

You will perform personal care, mobility assistance, nutrition support, and light housekeeping while following care plans. Indiana CNA/HHA certification is required, with CPR/First Aid preferred.

Qualifications

• Active CNA or HHA certification in Indiana is required.
• Previous home health or related healthcare experience preferred.
• Reliable transportation and ability to travel to Nineveh, IN area.
• Strong communication and ability to work independently.
• Compassionate, patient-centered attitude with commitment to quality care.
• CPR/First Aid certification preferred.

Responsibilities

• Provide personal care including bathing, grooming, dressing, and hygiene support.
• Assist patients with mobility, transfers, and ambulation.
• Monitor and document patient health status, reporting changes to supervising nurses.
• Support patients with meal preparation and nutritional needs.
• Perform light housekeeping and maintain a safe home environment.
• Deliver companionship and emotional support to patients and families.
• Follow individualized care plans and adhere to home health protocols and procedures.
