Job summary

ELITE HOME HEALTHCARE LLC is seeking a Home Health Aide to provide personal care services to clients in their home under the direction of a Registered Nurse. The aide is assigned by the RN or other qualified professional and helps maintain the client's comfort and safety.

Qualifications include formal certification training and/or a competency evaluation, strong communication skills, and being at least 18 years old.

Qualifications

• Formal certification training program completion and/or competency evaluation.
• Good verbal, written, and interpersonal skills.
• Must be at least eighteen (18) years of age.
• Ability to read, write, and follow a written Plan of Care.

Responsibilities

• Performs delegated nursing tasks as an extension of nursing services under RN supervision.
• Provides personal care activities to clients in their homes.
• Assists with household services essential to health care at home.
• Documents care and completes required client records daily as per agency policy.
• Maintains infection control practices and ensures a safe home environment.
• Communicates changes in client condition to the RN Case Manager and team.
