Job summary

ELITE HOME HEALTHCARE LLC in Indiana seeks a Home Health Aide to provide personal care under the direction of the Registered Nurse, supporting clients in their homes. The role requires completion of a formal certification or competency evaluation, strong communication, and the ability to read and follow a Plan of Care.

You will work as part of an interdisciplinary team, documenting care daily and maintaining infection control and patient safety.

Qualifications

• Formal certification training and/or a written skills test and competency evaluation completed.
• Good verbal, written, and interpersonal skills.
• Be at least eighteen (18) years of age.
• Demonstrated ability to read, write, and follow a written Plan of Care.

Responsibilities

• Performs delegated nursing tasks under the supervision of the Registered Nurse.
• Performs personal care activities.
• Performs household services essential to health care at home.
• Assists in the administration of medications ordinarily self-administered under RN supervision and Agency policy.
• Reports changes in client condition to the RN and follows Plan of Care reporting guidelines.
• Documents care provided and completes required client records daily per Agency requirements.
• Maintains infection control practices and a safe home environment.
• Communicates with the interdisciplinary team as requested.
• Maintains confidentiality in all aspects of the job.
• Attends in-service programs to meet compliance requirements.
• Performs related duties as deemed necessary.
• Ensures compliance with Occupational Exposure to Blood Borne Pathogens requirements.
