Job summary

PurposeCare is a leading home health and home care provider in the Midwest, delivering one‑on‑one care in clients' homes and communities. We are seeking dependable, compassionate caregivers to join our team, delivering personal care with dignity and respect.

Licensed HHAs/CNAs are encouraged to apply, and motivated candidates willing to become certified are welcome. This role offers flexible scheduling (full‑time, part‑time, PRN) across our service area with ongoing training and support.

Qualifications

• Must have active HHA or CNA certification or willingness to obtain.
• Willingness to complete competency and skills assessments.
• Ability to follow written care instructions and document services.
• Reliable transportation and ability to travel to client homes.
• Ability to lift up to 25 pounds.
• Willingness to complete TB testing, background screening, and onboarding requirements.
• Compassion, patience, and professionalism with vulnerable populations.
• Ability to work independently in home settings.

Responsibilities

• Provide personal care support including bathing, grooming, dressing, oral hygiene, and toileting
• Assist with mobility, transfers, ambulation, and range-of-motion exercises per care plan
• Prepare meals and assist with feeding as needed
• Support medication self-administration through reminders (no independent med administration)
• Monitor and report patient condition and changes to the supervising nurse
• Take and record basic vital signs when trained and assigned
• Maintain a clean, safe, and orderly client environment
• Assist with linens and light client laundry as appropriate
• Use safety equipment properly (rails, supports, stools, etc.)
• Document care provided according to company and regulatory standards
• Travel reliably to patient homes and assigned visits across your service area
• Complete required in‑service training, including annual dementia education
