Job summary

MENTOR Adult Day Health in Worcester, MA is seeking a dedicated Home Health Aide (HHA) to provide one-on-one in-home care to clients with complex medical or rehabilitative needs. You will assist with daily living activities, monitor health, and document services while supporting independence in the comfort of the client's home.

Qualified candidates hold an HHA or CNA certificate, a valid driver's license and reliable vehicle, and are comfortable using mobile devices for documentation.

Qualifications

• High school diploma or equivalent; must be at least 18 years old.
• HHA or CNA certification required.
• Reliable vehicle with valid driver's license and ability to travel to client homes.
• Comfortable using mobile devices for documentation and record keeping.
• Pass a background check and health screenings.

Responsibilities

• Provide in-home support with daily living activities (ADLs) such as bathing, dressing, and grooming.
• Assist with mobility, transfers, and personal care.
• Support light housekeeping and meal preparation.
• Run errands or assist with grocery shopping as needed.
• Monitor health conditions and document services provided.
• Build positive, trusting relationships with clients and ensure a safe home environment.
