Job summary

Comagine Health in Albuquerque, NM is seeking a Home and Community-Based Services Assessor to conduct annual in-home assessments for adults via the Mi Via Waiver. This primarily remote role involves visiting participants in their homes, documenting daily living skills, behaviors, and support needs, and using multiple electronic systems.

You will manage scheduling, ensure HIPAA compliance, and collaborate with a team of assessors to support independence and community access for individuals

Qualifications

• Associates degree in social work, psychology, or related behavioral health field.
• 2 years of social service or relevant work experience.
• Valid, unrestricted driver's license and reliable transportation.

Responsibilities

• Manage in-home assessments across the Albuquerque region using your personal vehicle.
• Coordinate and schedule annual in-home assessments including outreach and follow-up calls.
• Conduct person-centered, face-to-face assessments in participants' homes.
• Navigate multiple technology platforms to document findings and close out cases.
• Ensure confidentiality and HIPAA compliance in all work.
• Communicate with supervisor and colleagues; participate in training and meetings.
