Job summary

Comagine is seeking a Home and Community-Based Services (HCBS) Assessor to conduct in-home assessments in the New Mexico region. This role focuses on supporting individuals receiving services through the New Mexico Mi Via Waiver, ensuring they have access to necessary community-based services.

As a member of a collaborative team, you will engage directly with participants, administer assessments, and document findings using various technology platforms. The position is primarily remote, offering a flexible work schedule to maintain a healthy work-life balance.

Qualifications

• 2 years of social service or nursing experience.
• Valid driver license with proof of insurance.
• Ability to pass a Motor Vehicle Record check.

Responsibilities

• Manage schedule to complete in-home assessments.
• Coordinate and schedule annual assessments.
• Conduct face-to-face assessments in homes.
• Document findings using multiple technology platforms.
• Ensure compliance with confidentiality and HIPAA.
• Participate in training and team meetings.
