HarmonyCares is seeking a Value-based Advance Practice Provider (NP/PA) to manage high-risk geriatric patients in-home in Dripping Springs, TX. This onsite role includes a $10,000 signing package, salary range of $135k-$155k per year, and a quality-based bonus.

Qualifications

• Licensed as NP or PA in the practicing state.
• Active CPR/BLS certification.
• Federal DEA license eligibility.
• Geriatric training/experience preferred.
• Board Certification.
• Must maintain a valid driver's license and good driving record.

Responsibilities

• Provide primary care to high-acuity homebound patients.
• Focus on patient-centered care and quality outcomes in geriatrics.
