The position offers a signing package of $10,000 plus a salary range of $135k-$155k per year, with additional benefits and a quality‑based bonus. Join a national network serving 85,000 high‑risk Medicare patients across 12 states.

Qualifications

• Licensed as NP or PA in the state where HarmonyCares Medical Group operates.
• Federal DEA License/eligibility.
• Geriatric training/experience preferred.
• Active CPR/BLS Certification.
• Board Certification.
• Must maintain a valid driver’s license and good driving record.

Responsibilities

• Provide primary care to home health patients, with focus on high-acuity geriatric cases.
• Practice medicine with emphasis on patient care over volume.
• Collaborate with interdisciplinary team to coordinate in-home care across markets.
• Deliver care Monday–Friday onsite in patients’ homes.

Responsibilities

The Advance Practice Providers we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.
