Job summary

ChristianaCare HomeHealth is seeking a compassionate Registered Nurse to provide one-on-one nursing care in patients' homes. You will build relationships with patients and caregivers, promote independence, and develop individualized care plans aligned with available resources.

You will coordinate with physicians and the inter disciplinary team to achieve optimal outcomes, manage discharge planning, and perform IV therapy with training to achieve proficiency within 90 days of hire.

Qualifications

• BSN is required.
• ADN candidates must obtain BSN within 3 years.
• Current Delaware or Compact RN license.
• Minimum 1 year RN experience.
• IV proficiency or willingness to train.

Responsibilities

• Deliver skilled nursing care and treatments to geriatric patients at home and in the community.
• Build trusting relationships with patients and caregivers to promote independence and quality of life.
• Develop and execute individualized patient-centered care plans.
• Coordinate care with physicians, caregivers, and interdisciplinary team members to achieve outcomes.
• Develop and execute discharge plans ensuring continuity of care.
• Perform IV therapy and related nursing functions; achieve IV proficiency within 90 days.
