Job summary

Blue Ridge Care is seeking an RN/LPN for a Part-Time PACE role delivering in-home nursing care to older adults in Virginia. The position supports autonomy, comprehensive assessments, and collaboration with an interdisciplinary team across homes and community settings.

The role emphasizes strong clinical skills, care planning, and caregiver education, with a Friday–Sunday schedule, 16–30 hours weekly, and applicable shift differentials.

Qualifications

• Current RN or LPN licensure valid in Virginia.
• BLS/CPR certification remains active.
• Minimum 1 year caring for frail or elderly individuals.
• Experience in home health, geriatrics, hospice, or community care preferred.
• Ability to work independently with strong communication within a team.

Responsibilities

• Conduct comprehensive in-home nursing assessments and ongoing evaluations.
• Perform skilled visits: medication management, wound care, injections, phlebotomy.
• Assess condition changes and communicate with the IDT promptly.
• Develop and monitor individualized care plans with the IDT.
• Educate participants and caregivers on disease management and safety.
• Coordinate transitions after hospitalizations or facility stays.
• Document care accurately in the EMR and participate in on-call rotation.
