Old Glory Home Health Care & Assistive Care in Central Virginia is seeking Registered Nurses to join a compassionate home health team dedicated to patient-centered care.

Our RNs perform skilled visits, patient assessments, care coordination, wound care, and post-hospital follow-up, with autonomy and meaningful impact in patients' homes. Qualifications

• Active Virginia RN license required.
• Current CPR certification required.
• Reliable transportation to patient homes.
• Valid driver’s license and auto insurance.
• Home health experience preferred.
• OASIS experience preferred for RN applicants.

Responsibilities

• Skilled nursing visits in the patient’s home.
• Start of care, recertification, resumption of care, and discharge assessments.
• OASIS documentation and care planning.
• Medication reconciliation and patient education.
• Wound care and chronic disease management.
• Communication with physicians, families, therapists, and the care team.
• Vital sign monitoring and IV infusions.
• Support for chronic disease management and transitions.
